Our Continuum of Care:

Hospice - Specialized health care services that focus on pain relief, symptom control, comfort and support for terminally ill patients. Hospice care can be given in a patient’s home including assisted livings or nursing facilities. A team that consists of a physician, nurse, home health aide, social worker, chaplain and volunteer provide care and support to the patient and family through the end of life. Hospice covers any medication related to the terminal diagnosis. A six month life expectancy is the guideline. Medicare, Medicaid and some private insurance pay for hospice care.

Hospice House - The Hospice House is a state-of-the-art facility that provides a comfortable home-like setting and 24-hour hospice care. Hospice House is desiged for patients who need 24-hour symptom management. Care is provided by a team of physicians, nurses, social workers, clergy, home health aides and trained volunteers. A plan of care is developed with input from the patient and family as well as the hospice team.

ADDITIONAL HEALTH CARE SERVICES

Garden View Respite Center- A residential facility that offers clients and caregivers options for brief or extended stays. The respite center is ideal for someone when their caregiver is on vacation or recooperating from surgery. The center’s warm, caring atmosphere feels like a home with inviting living rooms, outdoor patios and walkways and beautifully decorated client rooms. It is staffed by an experienced team of health care professionals.

PACE (Program of All- inclusive Care for the Elderly) - PACE is responsible for everything related to the client’s health care including physician, prescriptions, dental care, in-home services, transportation, meals and nutritional planning, speech, physical and occupational therapies, medical equipment and coordination of services and support needed for client to remain in their own home. This program is for people over 55 that are able to live SAFELY in the community, are eligible for nursing home care according to the Kansas long- term care placement score and live within the service area (Shawnee, Douglas, Jackson, Osage, Pottawatomie and Wabaunsee counties.)

Adult Day Center- The center is designed for adults over the age of 18 who require supervision or assistance with day-to-day activities and are no longer able to safely stay home alone. It is a safe, home-like atmosphere with a variety of activities designed to promote motivation, creative expression and self-esteem. The center is open Monday through Friday from 7:30 a.m. to 5:30 p.m.

Grief Counseling- The center provides support groups, individual session and children’s programs that provide support and insight regarding feelings of loss related to the death of a loved one. These services are available to anyone in the community, not just family members of hospice clients.

Palliative Care Services – Provides physician consultation in the community regarding comfort/symptom management. It provides the ability to pull in an interdisciplinary team approach as needed to address symptom management issues.

Midland Care serves the community out of offices in Lawrence, Ottawa, Council Grove and Topeka.
